How to Create Portal Forms

Collect responses using dynamic web forms embedded on a Portal.

Step 1: Go to Admin / Portals / Forms from the main navigation.

Step 2: In the Add Form section, enter your preferred Name and click Add.

Step 3: Click on the newly created form in the table.

Step 4: Edit the Form based on your desired options:

  • Name - enter a form name.
  • Description - enter a form description.
  • Default Language - choose the language for the form.
  • Reference - choose a unique code for the form (auto-generated).
  • Portal - choose the portal to host the form.
  • Status - draft, private, or public.
  • Redirect on Submission - after submission, redirect users to this link (can be external or to a Portal post).
  • Expires On - restrict submissions after this date (or leave blank for never).
  • Notify - comma-separated list of emails to receive form submissions.

Step 5: Add each desired Field using the Add a Field section. Once added, you can change the Field name, configure additional options, and drag-and-drop the fields positioning.

Step 6: Click on the Public or Private Link or QR Code to go to the form.

  • Form submissions will appear on the same screen under Submissions.
  • Include the link in any message templates (such as a Flow or Bulk Action). 
  • Forms can also be triggered automatically using the "Form created" trigger in Auto Messages.
