Simplified Footer Bar

The footer bar has been simplified with the following changes:
- Report Issue button is sticky (always viewable regardless of scroll position) in the bottom right
- Agency Setup link is the first link in the footer
- Billing and Console links will only show for Super Admin users
- Agency Name is shown in a larger font size
